Background Information

The Doris Daniels Outreach for Breast Reconstruction is a non-profit organization providing affordable breast reconstruction to medically underserved women in the North Texas area. The organization is a publicly supported corporation organized exclusively for charitable and educational purposes. Individuals must qualify for Outreach services by completing an intake form.

Financial Assistance Policy

The Client may be required to pay a portion of the cost, to be determined after a complete evaluation. The portion will vary from client to client based on the procedure recommended and income level of the client.

Release of Liability

Waiver of Claims: The Doris Daniely Outreach is a 501 (c)(3) non-profit corporation. Medical professionals donate their time on a voluntary basis. The services being provided might not be available but for the efforts of the Outreach volunteers. The individuals provided services are not obligated to do so, but are doing so on a voluntary basis. The fair market of the services the client will receive exceeds the amount she will be expected to pay.

The Outreach has the right, in its sole discretion, to refuse to provide service or withdraw service in the event it determines that misrepresentation regarding information on the client's intake form have been made. In addition, the Outreach may refuse or withdraw assistance for any other reason.
